APPLY TO REGISTER BIRTH AND OBTAIN BIRTH CERTIFICATE FOR A CHILD BORN IN MALE'

SERVICE DESCRIPTION


Any child born in Male’ must get there birth registered in Male' Municipality. This service is provided by Male’ Municipality. The following describes the procedure.

CRITERIA/PRE-REQUISITES
  • “Foolhumaa form”  issued when the time of birth must be submitted to Male’ Municipality within 7 working days.

PROCEDURE
  1. Submit “Foolhumaa form” with the necessary documents (listed in “Documents and forms required”) to the counter of Male’ Municipalityge  Aabadheeaai behey bai.
  2. Collect the “Ufandhuvahuge registry, Ufandhuvahuge certificate adhi Ufanvaa kudhin rahvesakah vumah edhey form” and if the applied name is not approved collect “Nan faas kurumah dhookuraa slip” from the counter. Fill these forms and submit it to the counter.
  3. Make the necessary payment and obtain the payment slip (the date and time to collect the birth registry and birth certificate will be mentioned in this receipt)
  4. Go to Male’ Municipality (Aabaadheeaai behey bai) on the date and time specified in the payment receipt and submit the receipt to the counter and obtain the birth registry and birth certificate.
DOCUMENTS AND FORMS REQUIRED
  • “Foolhumaa form”.
  • Filled "Ufandhuvahuge registry, Ufandhuvahuge certificate adhi Ufanvaa kudhin rahvesakah vumah edhey form" issued when “Foolhumaa form” is submitted to Male’ Municipality.
  • National Identity Card of child’s mother and father (copy)
  • Marriage certificate of child’s mother and father (original)
  • If child’s parents are divorced, divorce certificate (original)
  • If registering the child in a Male’ house, “Rahvehinge registry” of that house (original)
  • If requiring to register the child in “Dhafthar” , passport size photo of the child (2 photo)
  • Rf 1 revenue stamp.
FEE
  • Rf 20 to make the birth registry
  • Rf 20 to make the certificate
  • Rf 20 to make Dhafthar certificate if registering the child in Dhafthar.

    Fine / Penalties
  • If the “Foolhumaa form” is not submitted within 7 days of child’s birth Rf 5 will be charged as fine and Rf 1 for every increasing working day.
  • If the child is not registered within 90 days of the birth, Rf 100 must be paid when applying to register after this duration.
ADDITIONAL INFORMATION
  • Fine taken due to the late submission of Foolhumaa Form are charged by the law (7/92) and fine taken for not registering the child within 90 days of birth are taken under law (46/78)
  • Can submit one of the parent’s (either mother or father’s) marriage certificate when submitting the marriage certificate of child’s mother and father
  • If the child is born to a Maldivian who’s marriage has taken place in abroad, the marriage certificate submitted when applying for this service must be registered in a Maldivian Court
  • The name submitted for the child is a Muslim name or not will be identified if any person knowing the language of the name submitted or Ministry of Islamic Affairs sign on the slip stating that the name is acceptable for a Muslim to be called.
  • If there is any outstanding amount of fine, service of birth registering will not be provided to that person.
  • The child can be registered in Dhafthar or in a Male’ house, only if child’s mother or father is registered in Dhafthar or in a Male’ house.
